Objective
To work with State health agencies and other public and private nonprofit organizations in planning, developing, integrating, coordinating, or evaluating programs to prevent and control chronic diseases; and assist in monitoring the major behavioral risks associated with the 10 leading causes of premature death and disability in the United States including cardiovascular diseases.
Who Can Apply
- U.S. State Government (including the District of Columbia)
- Nonprofit Organization
- U.S. Territory (or Possession) Government (including freely-associated states)
Eligible applicants are the official State and territorial health agencies of the United States, the District of Columbia, tribal organizations, the Commonwealth of Puerto Rico, the Virgin Islands, Guam, the Northern Mariana Islands, the Federated States of Micronesia, the Republic of the Marshall Islands, the Republic of Palau, and American Samoa. Other public and private nonprofit community based organizations are also eligible.
