Objective
The various programs included in this assistance listing are: Rural Health Network Development supports integrated health care networks that address health care needs of targeted rural community. Rural Health Network Development Planning assists in the development of an integrated rural health care network, specifically for entities without a history of formal collaborative efforts. Rural Health Care Coordination supports rural health consortiums/networks by improving access, delivery, and quality of care. Rural Maternity and Obstetrics Management Strategies improve access to maternal and obstetrics care in rural communities through a network approach. Rural Health Care Services Outreach supports rural communities by expanding and enhancing health care delivery through a strong consortium. Small Health Care Provider Quality Improvement supports rural primary care providers in planning and implementation of quality improvement.
