Objective
The Integrated Maternal Health Services program fosters the development and demonstration of integrated maternal health services models, such as the maternity medical home, which is modeled after the patient-centered medical home. The purpose of the program is to improve maternal health outcomes in the United States by increasing access to quality, equitable, comprehensive care for pregnant and postpartum women who experience health disparities and have limited access to basic social and health care services.

Eligible entities for the Integrated Maternal Health Services program include any domestic public or private entity, including Indian tribes or tribal organizations, as well as domestic faith-based and community-based organizations.
