Community Integrated Service Systems (CISS)

Objective

The Community Integrated Service Systems program works to assist states and communities with building a comprehensive, integrated system of care to improve access and outcomes for all children, including children with special health care needs.

Who Can Apply

  • Unrestricted by Entity Type

Award preference is provided to areas with high infant mortality for CISS investments supporting the following investment areas: Maternal and infant health home visiting programs; Projects designed to increase the participation of obstetricians and pediatricians; Integrated maternal and child health service delivery systems; Maternal and child health centers; Maternal and child health projects to serve rural populations.

Investments supported under the CISS authority are designed to help states and communities build a comprehensive, integrated system of care to improve health care access and health outcomes for all children, including children with special health care needs. CISS investments are intended to develop and expand the following: Maternal and infant health home visiting programs; Projects designed to increase the participation of obstetricians and pediatricians; Integrated maternal and child health service delivery systems; Maternal and child health centers; Maternal and child health projects to serve rural populations; and Outpatient and community-based services programs (including day care services) for children with special health care needs whose medical services are provided primarily through inpatient institutional care.
