Health Systems Strengthening and HIV/AIDS Prevention, Care and Treatment under the President's Emergency Plan for AIDS Relief

Objective

This program supports the development and strengthening of the health workforce and health care delivery systems in countries supported by the President’s Emergency Plan for AIDS Relief (PEPFAR). Consistent with the agency’s statutory role in workforce development, education, training, and technical assistance, program activities focus on building local capacity, integrating health services into routine care, and strengthening systems that enable host country governments to manage and sustain U.S.-supported health programs. These efforts advance the America First Global Health Strategy by promoting country ownership, sustainability, and alignment with U.S. strategic and national security priorities. Objectives include: (1) Strengthen health care delivery systems by expanding access to quality services through workforce support, integrated care, and appropriate health technologies. (2) Support international partnerships that improve population health programs and integrate them into routine health service delivery. (3) Advance U.S. strategic and national security priorities by reducing preventable disease, disability, and death in the United States and globally. Health Systems Strengthening Programs funded through cooperative agreements: (1) Leveraging Health Service Approaches for Sustainable HIV Epidemic Control; (2) Global Reach II; (3) Quality Improvement Solutions for Sustained Epidemic Control; (4) Capacity Building for Sustainable HIV Services. Non-Competing Continuations for FY25: (1) Leveraging Health Service Approaches for Sustainable HIV Epidemic Control (HSEA): $8,000,000  Supports HRSA’s statutory role in strengthening health workforce capacity and health care delivery systems by improving access to quality HIV services. Activities use timely data, appropriate technology, and service delivery improvements to strengthen routine health systems, expand workforce capacity, and support sustainable population health outcomes in PEPFAR-supported countries. (2) Global Reach II (GRII): $3,750,948 Advances HIV epidemic control by strengthening treatment delivery within national and regional health systems. The program focuses on workforce support, service integration, and system management to improve continuity of care and support country-led program implementation. (3) Quality Improvement Solutions for Sustained Epidemic Control Project (QISSEC): $2,684,125 Supports continuous quality improvement across the HIV care continuum. Activities strengthen provider skills, improve service delivery processes, and enhance patient outcomes while reinforcing routine health system functions. (4) Capacity Building for Sustainable HIV Services – Global (CBP): $11,563,395 Builds workforce and health system capacity through education, training, and technical assistance. The program applies proven U.S. practices to strengthen service delivery, improve patient outcomes, and support the transition of U.S.-supported HIV services to host country governments. Competing planned for FY26: Global Reach III (GRIII). (FY26 Funding TBD) Will support AFGHS priorities by strengthening the health workforce, integrating HIV services into routine care, and promoting practical service delivery models. The program will focus on sustainability, country ownership, and improved health outcomes in alignment with U.S. strategic and national security priorities.
