Organizations should meet the following criteria: - Organization mission should be aimed at promoting health, well-being, and quality of life in New Jersey communities. - Be located in and serve Horizon Blue Cross Blue Shield of New Jersey's service area in New Jersey. - Have demonstrated experience with the activities proposed in the funding proposal. - Should not duplicate the work of other organizations already supported by the Foundation within the same geographic area. - Be able to identify specific, measurable, short- and long-term objectives. - Have a current and satisfactory financial audit.
