How to Apply

The purpose of The Horizon Foundation for New Jersey is to promote health, well-being, and quality of life in New Jersey communities. The specific objectives listed below are stated to support this purpose. The Horizon Foundation for New Jersey only considers requests submitted by organizations that have a tax-exempt classification under Sections 501(c)(3) of the U.S. Internal Revenue Code. Organizations should meet the following criteria: - Organization mission should be aimed at promoting health, well-being, and quality of life in New Jersey communities. - Be located in and serve Horizon Blue Cross Blue Shield of New Jersey's service area in New Jersey. - Have demonstrated experience with the activities proposed in the funding proposal. - Should not duplicate the work of other organizations already supported by the Foundation within the same geographic area. - Be able to identify specific, measurable, short- and long-term objectives. - Have a current and satisfactory financial audit.
