The application will request the following: the UTU auxiliary member's name, address, e-mail address, phone number and lodge number. The relationship of the student applicant to the member. The applicant student name, address, e-mail address, phone number and date of birth. The name of the college they are attending. Their rank in college (i.e. freshman, sophomore, etc.). Signatures of the related auxiliary member, the student applicant and the auxiliary member's lodge treasurer.

Restrictions

Scholarships are awarded to relatives of the Smart Transportation Division Auxiliary members. The applicant must be at least a high school senior and under 25 years old at the time of application. The applicant must be enrolled full time in an accredited college or university with at least a 2.0 grade point average.
