Applicants must be prepared to include with their proposals the following documents: a copy of their IRS Determination Letter of 501(c)(3) status and tax identification number; a cover letter identifying the objective of the applicant's proposal; a project budget; a list of funding sources for the proposal, if beyond support from the Foundation; a list of the applicant's Board Members and a detail of Board Participation; and information regarding any relationship between the applicant and Bluestone Bank.
