Required attachments include a grant application cover sheet, a uniform organizational operating and project budget, a list of current and projected funders of the organization and project, a list of board members including affiliations, a copy of the organization's IRS 501(c)(3) determination letter, the most recent audit and 990, and a list of staff with qualifications.
