How to Apply

Written proposals of no more than five pages are required providing a brief description of the organization's history, purpose and activities, statement of the need for support, the Hyde Park/Kenwood Park population it serves, how the project will promote economic development (if applicable), and how the planned project will address the stated need. Required attachments include a grant application cover sheet, a uniform organizational operating and project budget, a list of current and projected funders of the organization and project, a list of board members including affiliations, a copy of the organization's IRS 501(c)(3) determination letter, the most recent audit and 990, and a list of staff with qualifications.
