Grant applications are available by mailing a request to the foundation. Application must include: - Name of organization - List all of the organization's/institution's managers, including a short biography detailing their background, experience and qualifications - List the organization's/institution's history of community service and a history of its activities/programs - Describe in a letter the goals and objectives of the organization/institution and how the funds will be utilized - Explain the organization's/institution's financial needs. Provide information or financial statements that will confirm the organization's/institution's financial situation - List three (3) references that can attest to the organization's/institution's managers, programs, activities, practices, etc. Include each reference's phone number, address and email - Attach a copy of the organization's/institution's IRS determination letter as a 501(c)(3) organization/institution - Have the applicati…

Restrictions

All organizations and institutions receiving grants from the foundation must submit a written report describing the use of funds, how the organization or institution complied with the terms of the grant, and the progress made by the grantee toward achieving the purposes for which the grant was made. The grantee shall make such reports as of the end of its annual accounting period within which the grant or any portion thereof is received and all subsequent periods until the grant funds are expended in full or the grant is otherwise terminated. Such reports shall be submitted to the foundation no later than six (6) months after the close of the grantee's annual accounting period. When the grant funds are fully expended, the grantee must make a final report with respect to all expenditures made from such funds (including salaries, travel, and supplies) and summarize the progress made toward the goals of the grant.
