An individual or organization desiring a grant from the corporation shall submit the following to the grant committee: (i) general history and description of the applicant (including financial situation of the applicant) (ii) all contact information of the applicant; (iii) a detailed description of the purpose for the grant; (iv) the amount of funding requested, (v) the time or times when funds are to be provided by the corporation; and (vi) a list of other sources of funding for the request detailing amounts received or committed.
