A letter of request detailing the tax exempt purpose of the entity and the project being undertaken; who will benefit from the project; other entities who will be participating in the project; amount that the requesting entity will be contributing to the project and the amount which the entity will be requesting from this foundation. In the narrative describing the project, the entity should state the start-up date of the project and the estimated completion date of the project. The entity is to inform the foundation of the success of the project and provide appropriate pictures. In addition to the letter of request, the tax exempt entity should enclose the following documents: - List of the entity's current officers and board of directors - Copy of entity's 501(c)(3) determination letter - Entity's last year-end financial statements - Entity's last Form 990 or Form 990-EZ submitted to the IRS
