Application materials and instructions can be found online at: https://www.northeastonsavingsbank.com/more-resources/community-giving Interested applicants must complete the donation application available on the above-listed website and must be prepared to submit the following materials: - A signed W-9 Form - A copy of their 501(c)(3) IRS Designation Letter - A formal grant request letter - Any documents that can be used for Low-to-Moderate (LMI) verification to show that the organization or program you are requesting funds for serves the LMI community Examples may include an applicant's annual report or brochure that spells out the income demographics the applicant serves, and/or a letter from the applicant's Executive Director on official letterhead stating the percentage of low-to-moderate income individuals benefited by the applicant's programs and services.
